YMCA Community Integrated Health is the effort to strengthen the linkages between traditional healthcare and community-based prevention strategies in order to help individuals prevent, delay, or live better with chronic conditions. Community integrated health:

  • Increases access to care
  • Lowers costs
  • Prevents and addresses chronic disease
  • Reduces the effects of some social determinants of health

Blood Pressure Self-Monitoring (BPSM)

The YMCA’s Blood Pressure Self-Monitoring Program (BPSM) supports adults with hypertension in lowering and managing their blood pressure.

This four-month program focuses on practicing home self-monitoring of blood pressure readings and individualized nutrition education for better blood pressure management.

With the support from a trained Healthy Heart Ambassador (HHA), participants will aim to:

  • Measure and record their blood pressure at least two times per month
  • Learn how to properly measure their blood pressure
  • Engage with Healthy Heart Ambassadors support resources
  • Participate in monthly Nutrition Education Seminars

Participants MUST meet all of the following requirements to participate:

  • Must be 18 years or older
  • Must have been told they have high blood pressure (hypertension)
  • Must not have experienced a recent cardiac event
  • Must not have atrial fibrillation or other arrhythmias
  • Must not be at risk for lymphedema

FREE Loaner Cuffs are available to registered program participants for all four months. Cuffs must be returned within ONE WEEK of program completion.

Y Members: $45/month

Community Participants: $75/month *includes a FREE Y membership for duration of enrolled program!

Thanks to a collaborative partnership with the Rock County Public Health Department, our Y is proud to offer the Loaner Cuff Program FREE & OPEN to Rock County residents 18+ that meet qualifications. In addition, all participants receive a FREE 1-month Y membership because we want to walk alongside you in your wellness journey.

What is self-measured blood pressure (SMBP)?

SMBP is when you measure your blood pressure outside of the doctor’s office or other health care settings.

Why do I need to measure my blood pressure if it was already measured at the doctor’s office?

SMBP allows you to measure at different times throughout the day and over a longer period of time, helping your doctor get a more complete picture of your blood pressure.

How does SMBP help me with my health?

By using SMBP, you and your care team can come up with a treatment plan to better control your blood pressure. This can prevent more serious health problems.
